Revision Anterior Cruciate Ligament Reconstruction: Causes of Graft Failure and Results of Revision Surgery

Abstract
The purpose of this study is to evaluate the causes of failure after anterior cruciate ligament(ACL) reconstruction and clinical results of revision ACL reconstruction.We reviewed 18 cases in 17 patients who had failure of graft after ACL reconstruction. The clinical results were evaluated according to the Lachman test, pivot shift test, Lysholms knee scores, and manual maximum anterior displacement test by KT-1000 arthrometer(MedMeric Inc, San Diego, CA).Causes of failure were technical errors in 15 cases(83%) and trauma in 3 cases(17%). Associated injuries at revision surgery were medial meniscus injury in 11 cases(61%), lateral meniscus injury in 6 cases(33%), chondral defect on lateral tibial condyle in 1 case and posterolateral rotary instability in 1 case. Excellent result was in 6 cases(33%), good in 6 cases(14%), and fair in 3 cases(17%).Most common cause of graft failure after ACL reconstruction was technical error. Successful result after revision ACL surgery could be obtained clinically.
